GENERAL SUMMARY:
The objective of the MDS Coordinator is to promote the physical and emotional well-being of nursing facility residents. The MDS Coordinator gathers information from residents and their families and the interdisciplinary care team during initial and periodic interviews.The core responsibility of the MDS Coordinator is to conduct patient assessments and interviews, coordinate and verify accurate and timely completion of MDS’s and RAP summaries.
Information from these assessments are used to formulate baseline and individualized care plans in collaboration with the interdisciplinary care team.

PRINCIPAL DUTIES:
- Completes all required MDS assessments (admission, 5 day, annual, change in condition, and discharge MDS) timely and accurately.
- Initiates baseline care plan upon admission in collaboration with interdisciplinary care team to meet regulatory requirements.
- Continually provides oversight and educates nursing and members of the interdisciplinary team regarding MDS protocols, accurate care planning interventions per State and/or Federal regulations.
- Responsible for timely and accurate electronic submission of MDS.
- Participates in family/resident care plan conferences.
- Tracks Medicare benefit days, validates daily Medicare census and coordinates the information with the business office.
- Facilitates weekly Medicare meetings and makes recommendations and collaborates with the interdisciplinary team focusing on resident goals and safe discharge process.
- Responsible for monitoring Casper Report and Quality Measures and identifies triggers and proactively collaborates with nursing and the interdisciplinary team to determine interventions and/or education needed to improve outcomes.
- Active member of the Quality Improvement/QAPI and Compliance Committees.
- Case management for non-Medicare short stay residents (HMO, Private insurance)
- Participates in Triple Check with the objective to review documentation for accurate billing prior to submission.
- Demonstrates consistent proactive collaboration and communication among IDT.
OTHER DUTIES AS ASSIGNED:
- Serves as Licensed nurse to provide direct patient care.
- Responds to actual and/or potential facility emergencies/disasters.
- May rotate schedule to cover a weekend shift as needed.
- Assists with dining as scheduled.
- Participates in administrative rounds as assigned.
- Other functions as directed by the Director of Nursing.
- Training staff on documentation requirements.
- Maintain current on changing regulations and reimbursement (PDPM) requirements
WHAT YOU WILL BRING:
- CA Licensed RN or LVN with a current license in good standing.
- Minimum 2 years’ experience as a MDS Coordinator in a Medicare Skilled Nursing Facility including MDS 3.0 experience.
- Must have strong knowledge and working experience with State and Federal regulations, the RAI (Resident Assessment Instrument) and working knowledge of PDPM criteria.
- Computer Proficient with Microsoft Word, Outlook and EMR system, preferably Point Click Care.
- Current certification as Resident Assessment Coordinator (RAC-CT), preferred.
- Must demonstrate detail orientation, capable of directing others and managing assessment schedules to maintain compliance.
- Excellent communication skills required.
- Flexible, decisive and responds quickly in high-pressure situations and completing work in a timely, accurate manner.
