Regional Quality Management Consultant (RQMC)
Reports to: Vice President of Clinical Operations (VPCO)
Qualifications:
· Previous experience as a Director of Nursing Services (DON) in long-term care.
· Previous experience as a Minimum Data Set Coordinator (MDSC), preferred.
· Strong verbal and written communication skills.
· Basic computer skills, preferred.
· Current California Registered Nurse (RN) licensure.
· Knowledge of Federal and State regulations and Reimbursement systems.
Position description:
· Promote person centered care initiatives through education and coaching of facility-based staff using policies and procedure as guideline.
· Consult and coordinate with healthcare care team members to access, plan, implement and evaluate patient’s quality care.
· Collaborate with facility team to develop person-centered care and serve on quality improvement teams.
· Assist in the development of processes and procedures to promote quality patient services and validate compliance consistent with professional standards.
· Analyze and monitor performance data to determine trends to improve person centered-care services.
· Demonstrates leadership ability and capability to work effectively with all levels of personnel.
· Evaluates compliance with current State and Federal regulations utilizing Critical Elements Pathway and systems validation tools.
· Evaluate MDS/RAI practices for consistency with Standards of Professional Practice, State Nurse Practice Act and compliance with company standards and regulatory requirements.
· Conducts facility reviews, to assess compliance with Federal and State regulations related to the MDS/RAI process and associated Rockport internal standards.
· Facilitates root cause analysis process and development of action plans for improvement of state quality measure program (i.e. CA QASP, 5 star program, etc.
Responsibilities and Duties:
· Conducts facility visits on a regular basis and reports any findings by utilizing the facility report form.
· Conducts entrance and exit conference meeting with the Administrator, DON, and the pertinent department manager.
· Participates in the interview process of the DON and/or the MDSC, as requested.
· Assists in developing and implementing facility systems in accordance with the policies and procedures.
· Conduct clinical rounds, interact with facility staff, residents their families and or responsible party to observe the provision of person-centered care based standards and nursing practice.
· Perform random interview of residents, responsible party and relevant staff members to evaluate patient care needs and or validation of care provided to the resident based on resident assessment and plan of care.
· Reviews the medical record for accuracy and completion of documentation and makes recommendation for the need of improvement.
· Random Review of Minimum Data Set (MDS) assessment as it relates to the RAI process for accuracy, timely completion, and development & implementation of the plan of care.
· Reviews Certification and Survey Provider Enhanced Reports (CASPER) reports and assists healthcare team members in developing systems utilizing Quality Assurance Performance Improvement (QAPI).
· Provides education and training based on identified areas of need.
· Supports the facility during regulatory compliance visits from outside agencies.
· Provides education in developing the plan of correction based on the results of the annual State and/or Federal Certification Survey.
· Validates the implementation of the plan of correction from the annual State and/or Federal Certification Survey.
· Perform and coordinate systems review when deemed necessary by the VP of Clinical Ops
· Attends and participates in interdisciplinary team meetings, as appropriate.
· Informs the VPCO and /or Chief Clinical Officer immediately for urgent concerns or problems.
· Evaluates efficiency in patient care by analyzing the utilization of nursing staff.
· Follow-up on trends assisting with development of specific action plans to meet practice standards and/or strategic plan initiatives.
· Conducts audits for MDS coding accuracy for newly hire MDS nurse and quarterly there-after.
· Keeps current on State and Federal regulations, assisting with interpretation as needed and identifying the impact of service delivery.
· Assist in development and implementation of facility specific quality improvement charters and action plans.
· Mentor facility leadership members in position accountabilities and execution of the IDT systems
· Assist if requested by the facility in the interview and orientation of new MDS coordinator.
· Participate as a key member of the Regional Operations team, communicates identified challenges and recommended solutions.
· Validate process that would demonstrate daily and weekly PPS meeting
· Assist in streamlining triple check process
· And other duties, as assigned.
Job Type: Full-time
Pay: $185,000.00 per year
